Do you accept insurance?

Yes, we will accept insurance; we are presently obtaining credentials from major insurance carriers. We also take fee-for-service.

Do I need to sign any forms before my appointment?

Yes, once you schedule your appointment, you will be sent a series of consent forms. Please complete all paperwork prior to your appointment.

Cancelation Policy

When booking a visit with us, cash payers will be asked to submit their credit card information. If you need to cancel your appointment, please do so at least 48 hours in advance, or your credit card will be charged in full. Payment is expected at the moment of service.

Will you provide a “Super Bill” or Billing Statement if I pay in cash?

We can issue you a billing statement or Super Bill, which you can then submit to your insurance provider for reimbursement. Although reimbursement is not assured, some insurance companies will reimburse up to 60% of the cost of the visit.

How do I get my medications refilled?

Prescriptions with enough medication to last until your next visit will be written. You are unlikely to require telephone medication refills. Requests for urgent refills may take up to 48 hours to complete.

Accepted Insurance Plans
